Intensive Case Management

CCLC's runs three Intensive Case Management Services programs with the goal of assisting  individuals and their family in overcoming barriers to reduce or eliminate the need for public assistance by:

      • Help make connections, gain employment and build a foundation 
      • Assist in navagating the SSI/SSD process "building the Case"
      • Assist individuals to comply with Social Service requirements and lift a sanction

 

Assessment & Planning

  • Provide self-assessment tools and options for areas of change, help establish and reach goals 
  • Assess physical and mental heath records to determine possible SSI/SSD case
  • Plan course of action, with client to prevent or lift a sanction from Social Services

Information & Referrals

    • Provide information, support and referrals to meet needs, such as GED classes, VESID, parenting, counseling, work sites, transportation, etc. 
    • Encourage involvement in our Connections Program, providing, support, work and social skills training
    • Gather clients history, and identify missing pieces to fully support SSI/SSD application 
    • Educate clients in public assistance requirements and provide support for follow-through  

Advocacy

  • We help clients and their family get the services that ythey need to be stable
  • We advocate and coordinate services with other professionals
  • Assist with transportation.

Referral Requirements

  • Referals come from Livingston County Department of Social Services cases loads, offering additional supports for clients with special needs.
