HCBS (Home and Community Based Services) are special services provided to people enrolled in NYS Medicaid who have specific medical and mental health needs. These services help people in Livingston County who are living with or in recovery from mental health or substance abuse disorders. The services help people to be more self-sufficient. The goal of the services is to help individuals to make use of skills they already possess in order to support growth. The most unique part of HCBS is that all services are provided right in the member’s home or preferred community location. The services include targeted help with:
- Psychosocial Rehabilitation
- Education and Pre-Vocational Support
- Family Support and Training
- Empowerment Services – Peer Support
- Intensive/Ongoing Supported Employment
People interested in getting more help in these areas of their lives need to be enrolled in a HARP (Health and Recovery Plan) by their MCO (Managed Care Organization).
Eligible individuals should contact their Health Home Care Manager for an assessment to be completed, which will further determine eligibility. The individual has a choice of agencies which can provide HCBS.
